ddx: (when not white) confluent and reticulated papillomatosis of Gougerot and Carteaud;  epidermodysplasia verruciformis

 

treatment:

topical Nizoral OR Nizoral 200mg PO QD X 2 days (warn patients that color may take weeks or months to return but they are still "cured" of the infection)
diflucan 150mg PO Qweek X 2 weeks
diflucan 300mg PO X 1 dose (repeat in 2 weeks PRN)

 

Malassezia Furfur:

(AKA pityrosporum ovale or orbiculare)        
normal resident of skin
produces azeleic acid
tinea versicolor; seborrheic dermatitis; pityrosporum folliculitis
