Apart from jaundice, both diffuse and circumscribed color changes may occur in chronic liver disease:

 

diffuse color change:

•muddy gray color largely due to basal cell melanin
•common in primary biliary cirrhosis;  generally mild in other forms of chronic liver disease

 

circumscribed color change:

•chloasma hepaticum

 

 

pathogenesis:

•? increased melanogenesis or decreased melanin degradation
•no increase in MSH
•hemochromatosis:  hemosiderin in the skin, and excess melanin
