aphthous ulcerations are common in patients with IBD, are thought to represent an autoimmune reaction
in IBD patients, the lesions occur only on mucosa not bound to periosteum

 

characteristic of Crohns:

linear ulcerations of the buccal vestibule,  a cobblestone appearance of the buccal mucosa, swelling of the gingiva and lips
biopsy of these lesions of the lips show non-necrotizing granulomatous inflammation

 

Cutaneous Crohn's disease:

AKA metastatic Crohns disease (= no direct connection to gut)
often umbilicus or groin region
all have granulomatous histology
